Patients now have online access to our Patient Portal!
In the Patient Portal, a patient may view his/her chart, view biopsy results, or enter patient forms directly into the Electronic Health Record. Each patient will be assigned a unique user ID and password at our office, and then will be able to log-in to our Patient Portal by clicking the link below. If you have questions about the Patient Portal, or want to set up your user ID, please call our office at (270) 663-1078.
If you prefer to fill out paper forms instead (or if you have not yet been assigned a username and password for the portal), both New & Established Patients can find patient forms below. You may print them at home to fill out at your convenience. Please bring these forms with you at your appointment time. Of course, you are always welcome to arrive 15 minutes prior to your appointment to fill these forms out in our office.
Prior to your first visit with Dermatology Center of Owensboro, we ask that each patient fill out our Patient Registration Forms and our Patient History Form. At subsequent visits, we only ask that you update your Patient History form once per year (or at any time your medication/health information changes). You only need to update the Patient Registration forms if your information changes (if you have a new insurance plan/card, if you move, change phone numbers, etc). If you have any questions about which forms you need to fill out, please do not hesitate to call us at (270) 663-1078.
If you would like to request your medical records from a previous practice (including Dr. Vessels' previous practice), you may also print a "Records Release" form. You may mail or fax this directly to the practice you would like your records from, or you can bring it with you to your appointment and we will fax it for you.
